A Age ≥ 65 years Older age increases cardiac risk
M Markers (cardiac) Elevated troponin or CK-MB
E ECG changes ST deviation ≥ 0.5 mm
R Risk factors ≥ 3 HTN, DM, smoking, FHx of CAD, hyperlipidemia
I Ischemia (angina ≥2 episodes in 24h) Frequent angina shows instability
C CAD known (≥50% stenosis) History of coronary artery disease
A ASA use in past 7 days Recent aspirin use without effect indicates risk

How TIMI Score is Calculated and Interpreted

The TIMI score assigns 1 point for each criterion met (maximum of 7 points). The higher the score, the higher the risk of adverse cardiac events (death, myocardial infarction, or urgent revascularization) within the next 14 days.

  • Score 0–2: Low Risk (5–8%)
  • Score 3–4: Intermediate Risk (13–20%)
  • Score 5–7: High Risk (26–41%)
